HCAlert serves as the backbone for telehealth and remote monitoring centers across Europe, allowing hospitals to manage chronic and post-operative patients under continuous clinical supervision.

Recognizing that conventional remote patient monitoring systems often feel fragmented or impersonal, Hope Care designed a new generation of platform—the Outpatient Monitoring Aggregator (OpMA). This system consolidates and harmonizes data from medical devices, remote monitoring tools, and Hope Care’s native disease programs (including Heart Failure, COPD, Diabetes, Multichronic, and Hypertension), as well as third-party systems, into a single, secure environment. Through intelligent triage, OpMA translates continuous data flows into prioritized insights—not to surveil patients, but to ensure that no clinical need goes unnoticed.

The Hope Care platform supports integration with more than 36 device manufacturers and processes 126 distinct data points. Beyond connected devices, it incorporates clinical surveys, video consultations, wearable sensors, and interoperability with external platforms, ensuring information flows efficiently and securely. Advanced algorithms continuously analyze vital signs, patient-reported outcomes, and behavioral data to identify early signs of deterioration. By triaging this information, the platform transforms raw data into actionable clinical cases, bridging the gap between technology and medical responsibility.

HCAlert also integrates natively with hospital information systems and electronic health records, enabling near real-time data exchange, workflow automation, and reduced manual documentation. This seamless integration allows healthcare professionals to focus on what truly matters—patient care.
